RHEL6互換環境で勝手にdevice mapperがmultipathを検知してしまい、(別マルチパスソフトウェア経由の)ファイルシステム作成時にエラーとなっていた。

# mkfs.ext4 /dev/sdb1
mke2fs 1.41.12 (17-May-2010)
/dev/sdb1 is apparently in use by the system; will not make a filesystem here!

下記で仮対応

# service mulitipathd stop
# /sbin/dmsetup remove_all

下記で本対応

# chkconfig mulitipathd off
/boot/grub/menu.lstのkernel起動コマンドラインに
nompath追加
# reboot
